    Fundamentally, I think you need to begin by controlling who can edit: there should be no anonymous editors, and the main moderator should be able to eliminate (ban) editors who are spamming/trolling/abusive. Even better would be a project that was by-invitation-only, with between a dozen and perhaps 100 contributors total. 
   1) start with an outlining/brainstorming phase 
   2) then define a cut-off point where that is over and there is only adding/editing text to follow that outlining phase. Edits which add subplot are OK, but you can't destroy the main plot thread that's been decided upon. 
   3) then another cutoff point and after that is only copyediting and bugfixing. You can fix grammar, spelling, punctuation, etc. but no removal of content or major content additions. 
   4) finally there is a cutoff point and the novel is declared done.
